Built in 1985, our 278-bed acute care hospital is located on Richmond Avenue just east of Dairy Ashford. HCA Houston Healthcare West is currently looking for a Registered Nurse (RN) Case Manager . Responsibilities: Under the general supervision of the Director, Case Management, the Clinical Case Manager is responsible for assisting in the implementation of the Case Management Program to ensure ongoing assessment of patient care delivery according to established standards. Coordinates patient care services provided in the Intensive Care/PCU Unit in collaboration with the Director on designated shifts Functions as a resource person for staff members and assists in necessary education of individual members of nursing staff with a focus on promoting clinical competence in the provision of critical care nursing. Assumes responsibility for the quality of nursing care provided by the Intensive Care/PCU Unit on designated shifts including the implementation of programs to improve this quality in collaboration with the Director. Ensures adherence to standards in the Intensive Care Unit/PCU based on Administrative Operational Standards, Nursing Administrative Standards, Human Resource Standards, and Intensive Care/PCU Unit Standards with collaboration with the Director, ICU/PCU, or Chief Nursing Officer in unusual situations. Demonstrates the facility's Standards of Performance, ICARE values, and Patient Promise. Assesses, monitors, records, reports, and follows through on symptoms or changes in patients' conditions. Creates and maintains accurate, detailed reports and records. Consults and coordinates with healthcare team members to assess, plan, implement, evaluate, and modify the plan of care, as indicated by the patient's responses and conditions. Delegates nursing tasks as appropriate and safe, but retains accountability for patient care Provides individualized education for the patient, family, and or caregiver throughout the continuum of care. Adheres to standards of safety and infection control, in the delivery of patient care. Demonstrates a thorough knowledge of and complies with all policies and procedures related to the performance of job duties Qualifications Qualifications: RN licensure by Texas Board of Nursing Minimum for 3 years of clinical practice with emphasis on leadership and management exp. Previous experience in critical care nursing with certification (CCRN) preferred. BLS and ACLS from the American Heart Association required. Current licensure in Texas as a Registered Nurse with Bachelor of Science in Nursing preferred. Experience in health care delivery systems with focus on Case Management or Performance Improvement preferred.
